The Arizona Board of Nursing (ABN) is responsible for issuing licenses to nursing assistants and nurses in the state. Nurses must be licensed to practice; however, there are several ways to apply for licensure through the ABN.

A nursing assistant is also known as a nursing aide. Nursing assistants are licensed in Arizona by endorsement. You do not have to take an examination, complete an internship or complete an education program.

- Locate and download the appropriate application form.
- Complete all sections of the application form, including education verification and payment of required fees.

You may be asked to apply for competency evaluation if:
- You have been out of practice for more than five years.
- You are applying for a new license in Arizona and have never been licensed by the AZ Board of Nursing (ABON).
- You have previously held an active license, but it has been expired more than two years.
To apply for Competency Evaluation, please follow these steps:

- Have an active and unrestricted license from another state at the time of application submission and throughout the licensing period if approved.
- Meet all of the education requirements for licensure in Arizona. If you do not meet these requirements upon approval of your endorsement application, you will be required to complete additional coursework prior to applying for a license as a registered nurse in this state by completing courses through an approved provider (exam preparation courses are not accepted).

- Search by Name: If you know the first and last name of the individual, then you can use this search option. Once you enter their information, it will display all available licenses held by that person. Clicking on any of those licenses will take you directly to that particular record where all information related to their license is displayed including contact info, expiration date and more.
- Search by License Number: In order to view someone’s specific record in AZ Board Of Nursing’s database, they must have an active license number associated with their account in our system. You can use this function if you’re looking for someone who has recently obtained their nursing credentials or if there was some sort of administrative error made during registration which resulted in incorrect information being displayed for them on your screen when performing other searches like “Search By City” or “Search By Status”.

If you are licensed in another state, or if you hold a current, valid license from another state, but have been inactive for less than 5 years:
- You must apply for reinstatement by submitting an RN Reinstatement Application (PDF).
- You may be required to complete additional requirements as determined by the Board’s review of your application.
If more than 5 years have passed since your last renewal date:
- You must complete all requirements for initial licensure again before being issued a new Arizona license.

A copy of a diploma or certificate showing the completion date and graduation date must be attached to this form, as well as documentation verifying that you completed your nursing education at an accredited program. The documents must be official, notarized, and provide proof that you graduated from an NLNAC accredited program.
10 Please use this application if you graduated from an international nursing program and wish to apply for initial licensure as a registered nurse or licensed practical nurse in Arizona by examination. Your education must first be evaluated by one of our approved evaluation services before applying for approval of your coursework in Arizona.
