If you want to check the status of your nursing license, there are a few things you can do. First, you can look at the nursing board website for your state or province. If you don’t see it on there, try calling them directly and asking about it. If all else fails, try checking with an agency like NurseAdvice.com or another source that specializes in helping nurses find jobs and make connections with other nurses from different states or provinces around the world.

• Online: Log in to “Texas Health Care Provider License Verification” at http://hcplive.dshs.state.tx.us/hcpverify/. Click “Search License Status” on the right side of the screen, enter your information and submit it to see if your license is valid or expired, inactive or surrendered as well as any pending disciplinary actions against you (for example, if you have been reported for practicing without a license). 4. If you have recently changed your name, please send a copy of the legal document that reflects your name change (e.g., marriage certificate, divorce decree) with your renewal application.

The BNE will not accept a name change letter for the purpose of license verification.

5. The BNE does not accept faxed renewal applications. Paper renewal forms will be returned to the applicant for completion of all required information and resubmittal of a complete paper application, which may delay the renewal process.

To complete a renewal application, you must provide all of the following information:

• your name and address
• social security number or other personal identification number
• license number(s) to be renewed

If you conduct business under a fictitious name, you must provide the fictitious business name and tax ID number. If there is any change in address or telephone numbers since last renewal period, please submit an updated copy of the form.

6. The BNE does not accept foreign transcripts or diplomas for licensure by endorsement. Foreign-educated nurses must apply for an Initial License and meet all examination requirements for Texas licensure as a registered nurse (RN) or licensed vocational nurse (LVN). Please note that foreign educated nurses must also meet all requirements of their country’s education ministry to be successful graduates and eligible to practice as nurses in their native countries in order to be considered for licensure in Texas.

If you are a foreign-educated nurse and your transcript is not in English, you must have it translated and evaluated by a credential evaluation service.
