The University of Iowa My Chart Login allows you to access your health record, view lab results, make appointments and add medications. If you are a patient of UI Health Care, please use this site to login.

Section: Under the My Chart tab on the left side of the page, click “Login”. Enter your username and password in the fields provided. Click “Sign In” to continue.

Section: On the next page, click on “Continue”. You will be taken to a page where you can choose which type of user ID you would like (patient or student). Choose one and fill out any additional required information for that user type — for example, if it is a student ID then enter their birthday and last name as shown in their passport. There is also an option to create a new patient account if this is not your first time logging in to My Chart!

When you click on the Sign Up Now button, you are taken to a page where you can create an account. You will have to enter your personal information in order to establish your identity. You must also provide us with your email address, password, and other information as required by law. You may choose any username or password you wish – they do not need to be unique or memorable (e.g., “password123”). Once this information has been submitted, we will send an email confirming that your account has been created successfully!

On the next page, you will be asked to answer how you would like to verify your identity. Choose any one option that fits you best and click on Next button.

You can choose to verify your identity using your phone number, email address or text message. You will receive a text message with an activation code for each of these options.

Enter the code in the box provided and click on Next button when done with this process

Enter the details required in the form and click on Submit button to receive an activation code.

• Create a username and password.
• Enter your contact information. This includes your first name, last name, date of birth, phone number, email address and home address.
• Enter your work address if different from the one above.
• Provide emergency contact information for yourself or another person who can be reached in case of an accident or emergency situation while at school activities such as sporting events or field trips.
• Provide insurance information that covers you while enrolled at the University of Iowa (if applicable).

After receiving the activation code over phone/email/text message, enter it in the text box and click on Submit button to activate your account.
